The Greens at Cannondale and Wilton Meadows Health and Rehabilitation Center are both located on a stunning campus nestled on ten acres of landscaped grounds in Wilton, CT. For over 15 years we have been providing our residents with exceptional care in an innovative and elegant environment.
The Greens at Cannondale is the area’s premier assisted living and memory care community. Our purpose is to enhance the quality of life for our aging residents. We develop strong relationships with our residents and their loved ones, giving all those involved peace of mind and the security that The Greens at Cannondale is the right place to call home.
Wilton Meadows is a 148-bed rehabilitation and long-term care facility that provides 24-hour nursing care to individuals with a wide range of complex medical conditions. We provide short-term rehabilitation services that help you regain function and recover from surgery or illness. Our long-term care services provide for the full personal and healthcare needs of your loved one.

The Greens at Cannondale and Wilton Meadows were the vision of builder and owner Fred Rzepka who recognized the need for a senior care community in the heart of Fairfield County. Fred designed his flagship Wilton Campus in order to create a place for seniors to go and live out the next chapter in their lives with joy and dignity, surrounded by warmhearted caregivers.
Without the constraints of a corporate “cookie-cutter” culture, the Wilton Campus has the flexibility to customize solutions to seniors and create a family-like culture among its staff. This has led to a true relationship-centered culture with unmatched staff longevity, exemplary customer service, and the highest level of quality possible – you can just feel the difference throughout the campus. The campus is still family owned and operated, and Fred regularly visits The Greens at Cannondale and Wilton Meadows to ensure his “gem” is serving local seniors as it was designed to do.
