What is a Skilled Nursing Facility?

A Skilled Nursing Facility (SNF) is a licensed Medicare and Medicaid facility with the main service offering being short term rehabilitation.
As a skilled nursing facility, we provide skilled nursing care and/or rehabilitation services to empower people to achieve their highest level of recovery and participation in life through excellence in rehabilitation. Our goal is to help individuals get back on their feet and back home.

Two Types of Skilled Nursing Care

Short-term care

Our main service offering is short term rehabilitation. If you have experienced a stroke, an orthopedic surgery, fall, or need help with a medically complex diagnosis we are here for you.

Long-term care

We also offer long-term nursing care and rehabilitation. Our SNFs offer hospice services for end of life and support to our residents and family members.

Skilled Nursing Services

As a skilled nursing facility, we provide 24/7 care. We are staffed with trained medical professionals, such as in-house physicians, licensed nurses, physical, occupational and speech therapist and more.

Skilled Nursing

Our SNF provides extensive care services, such as intravenous feedings, blood pressure monitoring, medication injections, post-surgical care, wound care, continence care, diabetic and oxygen management.

Rehabilitation

Our skilled nursing facility has a designated gym to provide physical, occupational, and speech therapy. We have the necessary equipment and technology to help you achieve your highest level of recovery.

Our healthcare team includes registered nurses, Licensed Practical Nurses, licensed therapists, CNAs, Case managers, Activities staff, Minimum Data Set coordinators, Physicians, Nurse Practitioners, wound care nurses and Dieticians.

Services & Amenities

Our facilities offer 24 hour care, resident and family care meetings, daily activities, food service, daily housekeeping, laundry, therapy programs, social services, and wound care services.
 
Our programs promote social and emotional well-being, such as our Music Therapy program, designed to elicit different responses from the brain using music, and our Low Vision Program, intended to help patients to make the most of their remaining vision by continuing to participate in daily activities and maintaining independence.

Skilled nursing facilities are specifically designed for seniors who require 24-hour medical supervision due to physical or mental conditions that leave them unable to care for themselves.
